Registration & Forms

Enrolling is easy in two basic steps:

1. Fill out the Registration and Information Form.   Registration and Information Form (PDF)  
Stop by our office for a registration packet or call us at 717-764-5603.  Or you can fill out the registration packet request form and we would be happy to mail a registration packet to you.

2. Pay the Registration Fee.   All new applications must be submitted with the registration fee.
